Children and Family Services - Notice of Intent - Notice of Work PCC Grantt Care

Services are to be delivered articulating strength-based philosophy that ties individual needs to specific interventions. Such as gains in measurable skills, increased productivity and pro-social behavior, improved functioning, attendance at school/academic success and a stable living arrangement in the community. Service outcomes will include:

• Enhance safety
• Increase pro-social behaviors
• Connection with extended family members
• School based functioning that supports current cognitive ability
• Increase capacity for healthy relationships
• Increase social and community connection
• Improved child development - healthy attachments, physical and intellectual development, and social and emotional health
• Permanency (family preservation, family reunification, other permanency home such as adoption transition to independence/adulthood)
