Health Canada requires a lease agreement for 103 units of
Hospital Grade Glucose Meters for 5 years including all test
strips and control solutions. At the end of the lease
agreement, Health Canada may return the equipment to the
supplier at no charge, or must have the option to extend the
lease period by two 1 year periods.
The leased equipments must include the Glucose Meters and all
necessary accessories including but not limited to: battery
pack, base unit, power supply, linearity kits, test strips,
control solutions and all other necessary supplies required for
the unit to be fully-functional as a whole. Health Canada
requires delivery of all units to Health Canada in Sioux
Lookout, Ontario within two (2) weeks from the date of contract
award.
The bidder must demonstrate that they meet every mandatory
evaluation criteria in their bid by submitting supporting
documentation. Failure to provide supporting documentation by
bid closing date will result in the bid being deemed
non-responsive.
Few of the mandatory evaluation criteria are listed below.
Please refer to the RFP (ANNEX "C") for the complete list of
mandatory evaluation criteria.
1. The unit and strips must be of Hospital Grade and not
personal use retail product.
2. Must have a feature that can be pre-programmed by Health
Canada's administrator to lock out the unit if quality
assurance (QA) checks are not completed daily and can be
unlocked by performance of QA check.
3. The bidder must certify that the unit can be cleaned using
Health Canada's existing supplies such as Accel TB wipes
(0.5% hydrogen peroxide) and alcohol swabs (isopropyl alcohol
70%).
4. Glucometer must be equipped with rechargeable battery
pack(s).
5. Must provide at least one user manual hard copy with every
unit and an electronic copy with delivery of units to Health
Canada.
6. Must be drop resistant, must have durable casing and clear
protective cover on screen and must not have other openings
on unit aside from opening for strip insertion.
A bid must comply with the requirements of the bid solicitation
and meet all mandatory technical
evaluation criteria to be declared responsive. The responsive
bid with the lowest evaluated price will be
recommended for award of a contract.
The Bidder must submit pricing in accordance with Annex B -
Basis of Payment, in Canadian Funds.

procurement.
