SCOPE OF WORK :
Work of this project comprises of, the repair or replacement of
sanitary and storm sewers, laterals and manholes to the existing
Collins Bay and Frontenac Institution sites, located at; 1445
Bath Road Street, Kingston, Ontario, and further identified as
PWGSC Project Number R.033192.001
The Contractor shall perform and complete the Work within 20
weeks from the date of notification of acceptance of the offer.

Please note there will be an optional site visit ( Site Visit
"B" ) on the 19th of February 2013 at 09:00 AM at "FRONT
ENTRANCE" CSC Collins Bay Institution, 1455 Bath Road, Kingston
ON - K7L 4V9, to further familiarize the job site. This site
visit is intended for only those who attended the mandatory site
visit on 31st January 2013 at 10:00 AM.
Bidders who did not attend the mandatory site visit will not be
allowed to participate in this site visit " B".

The site visit for this project is MANDATORY. Arrangements have
been made for a tour of the
work site. The site visit will be held on the 31st of January
2013 at 10:00 AM. Bidders are to meet at "FRONT ENTRANCE" CSC
Collins Bay Institution, 1455 Bath Road Road, Kingston ON - K7L
4V9.
Bidders are requested to communicate with the Contracting
Authority, before 10:00 AM on 30th of January 2013 to confirm
attendance and provide the name(s) and number of people who will
attend. The representative of the bidder will be requested to
sign Site Visit Attendance Sheet at the visit.
Bidders who do not attend or send a representative will not be
given an alternative appointment. Any clarifications or changes
to the bid solicitation resulting from the site visit will be
included as an amendment to the bid solicitation. Contractors
must bring picture ID. No pictures will be allowed and no cell
phones can be brought into the Institution.
The site visit for this project is MANDATORY. The
representative of the bidder will be required to sign the Site
Visit Attendance Sheet at the site visit. Bids submitted by
Bidders who have not signed the attendance sheet will not be
accepted.
***Please Note: This site visit will serve as an opportunity
for contractors to walk around the site and view the area(s)
where the work is to be completed. Enquiries of a technical or
contractual nature will not be answered during the site visit.
Drawings and Specifications are now available for viewing and
downloading in a Portable Document Format (PDF) or in a standard
paper version.
Standard Terms and Conditions for this procurement are
incorporated by reference into, and form part of the Tender and
Contract Documents. The Standard documents are issued by Public
Works and Government Services Canada and may be viewed on the
MERX electronic Bulletin Board as part of the information

IMPORTANT NOTICE TO BIDDERS
IMPORTANT CHANGES TO GI01 "CODE OF CONDUCT AND CERTIFICATION OF
- BID" R2710T, GENERAL INSTRUCTIONS TO BIDDERS (REFER TO SPECIAL
INSTRUCTIONS TO BIDDERS SI01 3)
Bidders should provide, with their bid or promptly thereafter, a
complete list of names of all individuals who are currently
directors of the Bidder. If such a list has not been received
by the time the evaluation of bids is completed, Canada will
inform the Bidder of a time frame within which to provide the
information. Failure to provide such a list within the required
time frame will render the bid non-responsive. Bidders must
always submit the list of directors before contract award.
Canada may, at any time, request that a Bidder provide properly
completed and Signed Consent Forms (Consent to a Criminal Record
Verification form - PWGSC-TPSGC 229) for any or all individuals
named in the aforementioned list within a specified delay.
Failure to provide such Consent Forms within the delay will
result in the bid being declared non-responsive.
A COMPLETE LIST OF EACH INDIVIDUAL WHO IS CURRENTLY ON THE
BIDDER'S BOARD OF DIRECTORS. (SEE APPENDIX 2 OF THE "ITT"
DOCUMENT)
