Pursuant to GI01 of General Instructions to Bidders R2710T,
bidders must submit the
following as part of their bid:

a. a complete list of names of all individuals who are currently
directors of the Bidder and

b. a properly completed and signed Consent to a Criminal Record
Verification form

individual named in the aforementioned list.

The above should be completed and submitted with the bid by the
bid solicitation closing date. If the above is not completed
and submitted with the bid by the bid solicitation closing date,
the Contracting Authority will so inform the Bidder and provide
the Bidder with a time frame within which to meet the
requirement. Failure to comply with the request of the
Contracting Authority and meet the requirement within that time
period will render the bid non-responsive.

New 50 Man Living Units

Location:
Mission, BC - Ferndale Institution, 33737 Dewdney Trunk Road

Work Description:
Work under this Contract comprises construction of New 50 Man
Living Unit building, site work for building only, U/G
electrical duct bank from generator building to New 50 Man
Living Unit, connection to plumbing and communication/electrical
services and remedial work.

Delivery Date:
The work shall commence immediately upon official notification
of acceptance of tender by the Contracting Authority and is to
be completed within Fifty-Two (52) weeks.

Site Visit Date:
A site visit has been scheduled for 02 October 2012 @ 9:30 AM
PDST. Contractors are to meet at the PWGSC site office at 33737
Dewdney Trunk Road, Mission, BC - Ferndale Institution.
Contractors are to note that this is not a mandatory site visit
and is optional. However, it is recommended that contractors
visit the site prior to closing.

All contractors must wear a high visibility vest, which meets
WCB standards, for all site visits including a tender site
visit. No cameras, cell phones (of any type) or laptop
computers are allowed into institutions at any time.

To assist with bonding and insurance requirements, this project
is estimated to cost between
$10,000,000.00 - $11,000,000.00.
