Pursuant to GI01 of General Instructions to Bidders R2710T
R2410T, bidders must submit the
following as part of their bid:

a. a complete list of names of all individuals who are currently
directors of the Bidder and

b. a properly completed and signed Consent to a Criminal Record

individual named in the aforementioned list.

The above should be completed and submitted with the bid by the
bid solicitation closing date. If the above is not completed
and submitted with the bid by the bid solicitation closing date,
the Contracting Authority will so inform the Bidder and provide
the Bidder with a time frame within which to meet the
requirement. Failure to comply with the request of the
Contracting Authority and meet the requirement within that time
period will render the bid non-responsive.

Work Description:
Work of this contract comprises design detailing, construction
planning including environmental protection; the supply and
installation of a replacement precast 2.44 m x 3.05 m high
precast concrete culvert of the existing 2.44 m x 2.44 m wood
box culvert at KM 632.5 of the Alaska Highway in Northeast, BC.
Town of Fort Nelson is at KM 453 of the Alaska Highway.

Work also comprises hydrological checks of expected flows verses
the carrying capacity with a reduced section steel liner to be
supplied and installed with grouting in the existing 1.5 m
diameter wood stave culvert at KM 678.4 of the Alaska Highway.

The work is to be completed by November 1st, 2012.

Value of Project:
To assist with bonding and insurance requirements, this project
is estimated to cost between
$475,000.00 to $525,000.00.

services provider. Firms that elect to base their bid on tender
(solicitation) documents obtained from other sources do so at
their own risk and will be solely responsible to inform the bid
