The following is a summary of the statement of work for this requirement.
The Correctional Service Canada has a requirement to provide offenders with efficient and effective health services health services that encourage inmate responsibility, by promoting healthy reintegration and contribution to safe communities.
Objectives: The objective of the contract is to provide health care services to the clientele which will aid in managing their health concerns and providing preventative services in order that they can be healthy in the future
Deliverables: To provide up to a maximum of one optometry clinic per week with a clinic being 3 hours in duration for a maximum of 52 clinics per year, attend National/Regional Health Services conferences/meetings/training on an as “as and when” required basis and provide monthly invoices with dates and types of services rendered.
Expected Delivery Date: July 1 2014 to June 30, 2015 with 2 option years (1st option year July 1, 2015 to June 30, 2016 and 2nd option year July 1, 2016 to June 30, 2017).
