Nature of Requirements:
The following is a summary of the statement of work for this requirement.
The Correctional Service Canada has a requirement to provide psychiatry services to federal offenders for Beaver Creek Complex located near Gravenhurst, Ontario.
Objectives: To provide forensic psychiatric clinic/services to offenders at Beaver Creek Complex in accordance with the policies and guidelines of the Correctional Service Canada.
Deliverables: Each individual consultation, assessment or treatment shall be recorded in the form of a Progress Summary and included in the Offender’s Health Care File
Estimated level of services is approximately 12 hours per month to be scheduled between Monday and Friday.
Expected Delivery Date: August 16, 2016 to August 15, 2019 with the option to renew annually for up to two (2) additional one-year periods.
